Hold the lighter horizontally by the lid, hinge facing down. 

Swing it open, and then quickly swing it back up. 

At the end of the second swing let the lighter swing through the gap between your middle-finger and thumb
Move your ring finger to the top corner of the lid, case of the lighter should just hang out between your 
middle- and index finger.
Now use your thumb to push the lid to the left. This will cause the case to go right.
Now as it is going right use your index finger to push the lighter into your palm. 
Ignite the lighter with your middle finger.
